Low Back:
1. Do you have pain, numbness or tingling below your knee?
2. Is your low back pain on both sides of your back or on the right or left side only?
3. Can you pinpoint your low back pain to a specific spot in your low back about the size of a quarter, on the left or right side?
4. Does your pain get worse when you bend backward?
5. If you sleep on your side with the painful side down, does the pain get worse?
6. Does the pain get worse going up steps?
7. Does the pain get worse when you cough or sneeze?
8. Do you have stiffness and pain when you get up in the morning?
9. Do you have groin pain?
!0. Do you have strong sharp pain in your low back, slightly below your waistline, on the left or right, that radiates down your leg but not past your knee?

6. Does your pain get worse if you tip your head back? ( look up to the ceiling)
7. Do you get headaches in the back of your head with your neck pain?
8. Did you ever have a whiplash type injury to your neck?
